NOTES

  1. This form must be submitted to and approved by the Corporation of the County of Prince Edward before approval of the Nutrient Management Plan.

  2. Approval and/or notification for any lands not within the County ofPrince Edward that are to be used in the calculation of the Nutrient Management Plan must be submitted to the County of Prince Edward.

  3. Copy of Manure Application Agreement must be submitted with this form.

  4. Please allow two (2) weeks for processing by the County.

  5. Form must be submitted in triplicate.

  6. It will be necessary to re-submit this information every fifth year prior to the fIrst day of March or at the time the land base ceases to be available, whichever occurs fIrst.
